Swing Bed Team


Klickitat Valley Health's Swing Bed Program offers short-term rehab and extended care for patients who are not ready to return home after a hospital stay, needing more time to strengthen and heal before they can step back into the routine of daily living. Our Medicare-certified program features newly renovated patient rooms and a high nurse-to-patient ratio for optimum care. As a hospital-based program, Swing Bed patients have access to 24 hour nursing staff, along with in-house therapists, pharmacy, and emergency services.

Who Needs Swing Bed Care?

Patients who need rehabilitation after orthopedic surgery, such as joint replacement
Patients recovering from a stroke
Patients receiving IV therapy or wound care
Patients needing to regain strength and mobility after an illness or injury

The Swing Bed Program offers two levels of skilled nursing care:

In Short-Term Rehab Care, Swing Bed patients play an active role in their care. They continue their medical or therapy treatments, and participate in activities and exercises designed to help them return home.

Extended Care is a long-term plan most commonly associated with nursing home care.

The Swing Bed Program offers patients the benefits of KVH's highly skilled healthcare professionals. As a team, they follow a written plan of care to help each patient achieve and maintain their optimum level of independent functioning. The team includes a KVH-based Provider, working in consultation with the patient's referring physician; the Swing Bed Coordinator; Skilled Nursing Staff; a Case Manager; an Activities Coordinator; Physical, Occupational, Respiratory and Speech Therapists; a Dietitian; and other professional referrals as needed.

Both Medicare and Medicaid pay for Swing Bed Care, as long as the patient meets certain criteria. Other sources of payment include many insurance plans and private pay. Insurance policies differ, so a Swing Bed Admissions Coordinator will verify coverage.

Medicare Qualifications

To qualify for Short-term Rehab Care, a Medicare recipient must have completed a 3-day acute care hospital stay and need continuing skilled care under a doctor's order. Skilled care is defined as treatments that require 24 hour licensed nursing or therapy oversight. Short-term Care is limited to a certain number of days per year. Medicare does not pay for Extended Swing Bed Care.

Admissions Criteria
Once the requirements for the payment source are met, the Admission Team will assess the "whole" person. This assessment evaluates factors such as behaviors, (yelling, wandering, or exit seeking), complexity of care, and family and social support. Admission to the Swing Bed Program is determined after all qualifications and criteria are met.
Medicaid Qualifications
To qualify for Short-term or Extended Swing Bed Care under Medicaid, an assessment must be done through the Department of Social & Human Services (DSHS) office. The assessment will assure that a person qualifies both functionally and financially, and meets the criteria for skilled intervention and oversight.

After a Swing Bed stay, some patients may need additional care to adjust to living at home. KVH Home Health provides in-home services to complete the recovery and transition process.
