Care Beyond Your Provider Visit

Your health does not stop when your appointment ends. At KVH, our care coordinators help you follow through on your care plan between visits. We work alongside your provider to remove barriers, answer questions, and connect you with the right resources, right here in Klickitat County.

Care Coordination

KVH’s care coordination team grew out of national healthcare reform efforts focused on better, more connected care. Care coordinators support your provider by extending care beyond the clinic visit, especially when you may need extra follow-up or community resources.

A provider visit is often short, and the days and weeks after the visit matter. If challenges come up, like transportation, cost concerns, or trouble understanding next steps, your care coordinator can help you work through them.

When your KVH provider sees that follow-up would be helpful, they connect you with a care coordinator. Your coordinator will reach out, learn what you need, and help you take the next step with confidence.

Chronic Care Management (CCM)

If you live with ongoing health conditions and need routine support, KVH offers Chronic Care Management. When you enroll, a nurse works with you, often by phone, to set goals, identify barriers, and build a care plan that fits your life.

CCM focuses on the time between visits, the days when questions come up, medications change, or symptoms shift. You are not expected to manage it all alone, and your KVH team can help you stay on track with a clear plan.

Medicare Annual Wellness Visits

Medicare Annual Wellness Visits help you create or update a personalized prevention plan based on your current health and risk factors. It is not a head-to-toe physical, it is a focused visit that looks for gaps in care and supports prevention.

At KVH, these visits are often completed with one of our nurses, and your primary care provider reviews the plan and any next steps. Medicare covers the Annual Wellness Visit for eligible patients, with no copay or deductible for the visit itself.

Advance Care Planning and Advance Directives

Advance care planning is a conversation about the care you would want if you ever could not speak for yourself. This can include choosing a trusted decision-maker and completing an advance directive, a legal document that records your wishes for future medical care.

Our nurses can provide information, answer questions, and help you complete your advance directive. This is a chance to make your wishes clear, so your family and care team have guidance if decisions ever need to be made quickly.
