Klickitat Valley Health is committed to providing public access to records maintained by the hospital district. Our Public Records Officer is available to assist you with submitting a request and obtaining public records.

How To Request Public Records

To request public records, please download the Public Records Request form here. Submit the completed form to our Public Records Officer using one of the options below:
