Palliative Care at KVH
When you are living with a serious illness or long-term health condition, you deserve support that fits your life. KVH palliative care helps you feel better, manage symptoms, and stay connected to the care you need, right here in Klickitat County.
Palliative care focuses on comfort, symptom relief, and quality of life for patients managing serious illness or long-term health conditions. Palliative care is not hospice or end-of-life care. We focus on relief, planning, and support while you continue your ongoing care and treatments.
Our team includes providers, nurses, pharmacists, and a social worker who coordinate care around your goals. We partner with your KVH provider and any specialists you may see.
Who Benefits From Palliative Care
Palliative care may help if symptoms are making daily life harder. It can also help when appointments, medications, or decisions feel like too much to handle alone. Palliative care is appropriate at any age.
If you receive primary care through KVH, talk with your provider about a palliative care referral.
How We Help
Palliative care helps with symptoms like pain, shortness of breath, nausea, poor sleep, and stress. We help you understand your choices and communicate with your care team so everyone is on the same page.
We also support family members and caregivers, because strong support helps patients do better.

- Listening, encouragement, and, if desired, prayer or reflection with a chaplain
In addition to continuing care with your KVH primary care provider, you will also have visits with our palliative care ARNP, Annie Stone, who works closely with your provider and specialists to support your care plan. A pharmacist with specialized training in geriatric medication management will review all home medications and supplements when you enter the program and periodically afterward to help ensure safety, effectiveness, and the best possible symptom control.
What to Expect
Your first visit with palliative care is time to talk, ask questions, and set goals. We will review your symptoms, your medications, and what matters most to you. Together, we build a care plan and coordinate next steps with your primary care provider.
How to Make a Referral
A referral is needed from your KVH primary care provider to start palliative care.
If you are the patient’s primary care provider:
- Request the palliative care referral form by emailing palliative@kvhealth.net or calling (509) 772-2691.
- Complete the referral form and fax it to (509) 773-0384.
If you are not the patient’s primary care provider:
- Call the patient’s primary care provider and ask them to complete the palliative care referral form, or
- Call the KVH Palliative Care office at (509) 772-2691 and we will help connect with the primary care provider.
Our team will then work with the primary care provider to complete the referral and contact the patient to schedule next steps.
This program is designed primarily for patients with Medicare and is typically billed through Medicare’s Complex Chronic Care Management (CCCM) benefit. CCCM supports patients living with multiple serious or long-term health conditions by covering care coordination, medication management, symptom support, and regular check-ins between visits. This allows our team to spend additional time helping you stay stable at home, avoid unnecessary hospital visits, and make sure all parts of your care are working together. If you qualify, our team will explain the program, review any applicable copays, and answer questions before enrollment.
