Who Will Make Decisions if You Cannot?
An advance directive is a legal document that lets you share your health care wishes ahead of time, in case you are ever unable to speak for yourself. It can also name a person you trust to make health care decisions on your behalf.
Having an advance directive helps your care team and your loved ones understand what matters most to you, including what treatments you would or would not want.
Common types of advance directives include:
- Living Will or Health Care Directive
- Durable Power of Attorney for Health Care
- POLST (Physician Orders for Life-Sustaining Treatment)
- MHAD (Mental Health Advance Directive)
Advance Directives at KVH
When you register for care, you will receive information about your patient rights, including your right to create an advance directive. You will also be asked whether you already have an advance directive.
KVH respects and honors advance directives within the bounds of the law and accepted medical practice. Your care is not affected by whether or not you have an advance directive.

If You Already Have an Advance Directive
If you have an advance directive, please bring a copy with you if possible. If your documents are at home or elsewhere, our team may ask you or your support person to help us obtain a copy so it can be placed in your medical record.
You Can Change Your Mind
An advance directive may be revoked at any time, either orally or in writing.
For Help Creating an Advance Directive
If you are interested in creating an advance directive, KVH staff can provide the appropriate forms and help you understand what the documents mean before you complete them.
For some patients, a POLST form may be recommended based on medical condition and goals of care. If appropriate, your care team can help initiate this process during your visit or hospital stay.
