Klickitat Valley Health is pleased to offer FREE sports physicals prior to the upcoming school year. We invite all middle school and high school student athletes to take advantage of this opportunity.

Date: Thursday, July 23rd
Time: 4 PM to 7 PM
Location: KVH Family Medicine, 317 Sanders Way Goldendale, WA
No appointment required
Instructions:
Download and print the sports physicals forms.
Parents/guardians must complete and sign the sports physicals forms.
Students should bring the entire packet to the appointment.
Parents do not need to be present, but the physical cannot be conducted without signed forms.
Please note that ASB purchases and IMPACT Concussion Baseline Assessments are managed separately by the Goldendale School District.
